Students interested in
participating on an interscholastic sports team must complete
a sports sign-up packet, which includes a total of five forms.
The forms must be completed by the athlete and his/her
parent/guardian.
The complete packet and individual forms may be downloaded by
clicking on the links to the right.

3. Interval Health History Form
4. Extra Curricular Code of Conduct Form
(must be signed every season)
5. Emergency Form/Consent Form
(must be signed every season)
Students who have not completed
the above process and have not been approved by the Health
Office will be considered ineligible for sports and can not
practice or participate until they are cleared by the nursing
staff.
If a student is involved in an
accident, which is related to his/her participation in
interscholastic athletics, the parent/guardian should first
notify the coach and then the school nurse. Upon his/her
returned from an injury, the student must have either a written
release or restriction from the attending physicians, which is
reviewed by the school nurse, a copy given to the coach and the
original is kept in the student's health records.
If athlete
meets all of the above requirements he/she will be eligible for
Duanesburg athletics.
1. School and Sport Physical Exam Form
Students must have a physical within 12 months of the beginning
of the sport season he/she wishes to participate in. Students
can go to his/her own physician for a physical or have one done
by the school physician.
Students needing sports physicals, school physicals (new
students and 7th and 10th grade students) and/or physicals to obtain
working papers should make an appointment with the high school
health office.
Please note: Physicals are by appointment only.
Students may also have physicals done by their own physician,
but the physical must be reviewed by the school nurse before a
student is eligible to attend practice.
2. Annual Health History Form
This form is necessary when a student is signing up
for their first sport of the year. The form should be completed
prior to the physical, signed by the student and their
parent/guardian and made available at the time of the physical.
If the form is not completed and
returned, the school physician may not give final approval for
the individual to participate.
3. Interval Health History Form
This form should be
completed in addition to the annual health history form if the
athlete's physical was completed more than 30 days before the
sport begins.
4. Extra curricular Code of Conduct Form
The Code of Conduct should be reviewed and signed by athlete and
their parent/guardian.
Students who have not completed the above process and have not
been approved by the Health Office will be considered ineligible
for sports until they are cleared by the nursing staff.
5.
Emergency Form/Consent Form
The emergency form/consent form must be signed and returned each sports season.
The coach will keep a file for his records in case of emergency.
