Health Notice: New
immunization requirements for school entry effective September 18, 2007.

New York
State Public Health Law Section 2164 mandates that schools not permit any
child to be admitted to school unless the parent submits a certificate of
immunization or proof from a physician, nurse practitioner or physician's
assistant stating the child is in the process of receiving the required
immunizations.
