Forms are available in the health office. We will be scheduling a
visit from the Bassett Health Clinic of Delanson some time in early
December for those students whose physicals have not been completed
by that time. Also, required by N.Y.S. is that all schools must have
on file prior to the child entering school a record of required
immunizations for all kindergarten students and new registrants.
Reminder: No student may take a medication in school or carry
medication in school without written permission from their
parent/guardian and a doctor’s prescription.
